*MUST HAVE HOME HEALTH CASE MANAGER EXPERIENCE* An experienced RN responsible for managing patients with a focus on care coordination and ensuring high-quality services. This role requires planning, organizing, and directing patient care for a designated caseload while collaborating with an interdisciplinary team. The RN will ensure effective management of patient visits and control expenses, making a significant impact on patient well-being. Home health experience is crucial for success in this position.
