Shift: 8 hour days, Description: Additional Information: Position Description: General job duties. Types of procedures required. Facilitating discharges to post acute care including SNF, AFH, Home Health and hospice. This is accomplished by assessing patients for right level of care, financial, and support. Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.)EPIC charting and sending of referralsRequired skills and minimum years of experience.Understanding and knowing how to do discharge planning. 2-3 years experience preferred.Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc)RN, preferred CCM or ACMShift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?) Day shift, mainly Monday thru Friday but could include rotation on weekends. No call.Floating to other units/departments. (If so, to which units/departments?)DCP covers all units of Med/Surg, Critical care and ED.Bed size/number of rooms for this unit/dept.TRMC is licensed for 110 bedsNurse to patient ratio of this unit. (nursing only)Cover a unit - usually 25-30 patients on a unit but not seeing all of these Dress Code:Business casualComments: Candidate needs skills in Discharge Planning Coordinator needs
