Role Summary: Acute Psychiatric Care in the Home:

This is an innovative and highly specialized role supporting a pilot Randomized Controlled Trial (RCT), the Behavioral Health Home Hospital (BHH) model, led by the Ariadne Labs Home Hospital team. The purpose of this trial is to test the feasibility and acceptability of delivering acute, inpatient-level psychiatric care to patients in their homes, a population currently excluded from standard home hospital programs.

The BHH Research Psychiatry Nurse is a crucial member of the clinical care team, providing in-person, hands-on hospital/inpatient-level psychological and clinical treatment in the community setting. The role is an excellent fit for a professional interested in research and advancing acute psychiatric treatment models.

Key Responsibilities and Patient Care:

The BHH Research Psychiatry RN works autonomously but in close collaboration with a multidisciplinary psychiatric team, which includes a Research Psychiatrist and Research Psychologist who conduct remote rounding.

Clinical Duties

  • Treatment Delivery: Provide in-person care a minimum of twice a day to patients in their homes or current place of residence during hospitalization.
  • Caseload: Manage a patient caseload of up to 4 patients at a time.
  • Assessment: Accurately perform initial and ongoing assessments, utilizing multiple data sources to determine actual/potential health problems and functional status.
  • Planning and Implementation: Independently plan and implement the patient's treatment based on established standards, collaborating with the psychiatric team for comprehensive care.
  • Medication Management: Responsible for dispensing medication directly to patients as part of the clinical care team.
  • Documentation: Ensure all patient care, education, and comprehension are documented thoroughly and timely in the electronic system, adhering to all program standards and regulatory compliance.
  • Recovery Focus: Utilize a Recovery Oriented Framework to identify and leverage patient strengths in the care process.

Collaboration and Logistics:

  • Coordination: Direct and coordinate all nursing care, collaborating with other professional disciplines, Home Health Aides, and Mental Health Specialists.
  • Scheduling: Adjust patient schedules routinely and communicate timely changes with the patient’s home-based clinical care team, the patient, and their families.
  • Safety/Support: Patients enrolled in the trial are required to have a caregiver present, and the program includes 24/7 remote monitoring and access to remote in-patient therapy groups.

This is an opportunity to be at the forefront of behavioral health innovation, helping to expand treatment access for patients with active acute psychiatric conditions.
