Description

WINTER / 13 WKS / 1st TIMER OK / Case Manager RN / DAYS

Shift: 6:00A - 6:30P (40 Hours/Week)

Requirements:
• First Time Traveler: Yes but 1st time travelers must be screened
• Ratios: Days 1:25
• Years of experience REQ: 2-year acute medical Care Manager experience performing Care Coordination/Discharge Planning
• Weekend REQ: typically one full weekend per 4 weeks, subject to dept. need
• CA state license

Shift:
Day 5x8-Hour (08:00 - 16:30)
