Travel LVN / LPN – Case Manager

Location: Tarzana, CA
Start Date: December 29, 2025
End Date: February 23, 2026
Contract Length: 8 Weeks
Schedule: Days, 5×8 (8:00 AM – 4:30 PM), 40 hrs/week
Employment Type: Travel/Contract

Position Overview

This role supports the Case Management department, focusing on care coordination, patient advocacy, discharge planning, and collaborating with interdisciplinary teams to ensure quality outcomes and continuity of care.

Key Responsibilities

• Assist in coordinating patient care plans and discharge needs.
• Communicate with providers, patients, and families regarding care options.
• Support case management documentation and regulatory compliance.
• Monitor patient progress and update care plans as needed.
• Work collaboratively with clinical teams to optimize patient outcomes.

Requirements

• Active California LVN/LPN license (must be current and unencumbered).
• Minimum 1 year of recent experience (Case Management or relevant clinical setting preferred).
• Ability to work a consistent Monday–Friday day shift.
• Strong communication and coordination skills.
• Must meet compliance: background check, drug screen, references, and standard immunizations.

Additional Details

• No remote option.
• Must be able to commit to full schedule.
• Some holiday coverage may be required.
• Floating within the system may be expected.
