Description

Job Overview
A Skilled Nursing Facility in Barrett, Minnesota is seeking an experienced Physical Therapist for a 13-week travel assignment. This role is ideal for a PT who is adaptable, comfortable with a variable day schedule, and experienced in post-acute and geriatric rehabilitation.

Responsibilities

Evaluate and treat residents in a skilled nursing facility setting
Develop and implement individualized treatment plans
Focus on improving mobility, strength, balance, and functional independence
Complete evaluations, daily treatment notes, and discharge summaries accurately and timely
Educate residents and caregivers on exercises, mobility strategies, and safety
Collaborate with nursing staff and interdisciplinary care teams

Requirements

Active Minnesota Physical Therapist license
Skilled nursing or post-acute experience preferred
Strong geriatric rehabilitation skills
Ability to manage a full caseload independently
Strong communication, organization, and documentation skills
