Location: North Region Scheduling (Service area: Chelsea, East Boston, Revere, Winthrop)

A Facility in Newton, MA is seeking for Home Care RN, a highly skilled and compassionate Registered Nurse (RN) Float for a Long-Term Assignment (LTA) in our North Region. This role is designed for a nurse who thrives in a dynamic environment, providing high-quality care to patients in their homes while managing a diverse caseload.
 

Assignment Overview:

• Contract Duration: 02/08/2026 – 05/09/2026 (13 weeks)
• Shift: Day Shift (08:00 – 16:30)
• Schedule: Monday – Friday; Weekend rotation once every 6 weeks
• On-Call: None

Key Responsibilities:

As a Home Health RN, you will deliver care centered around the organizational philosophy and standards of community health practice. Your duties include, but are not limited to:

• Patient Assessment: Evaluate patient conditions and environments to develop and implement comprehensive care plans.
• Case Management: Lead the coordination of care for a designated patient population, ensuring interdisciplinary communication and adherence to MGB Home Care policies.
• Clinical Care: Perform admission processes, wound care, medication management, and other skilled nursing interventions.
• Education: Provide ongoing support and clinical education to patients and their families to promote self-management and safety.
• Documentation: Maintain thorough, accurate, and timely electronic documentation in accordance with agency standards.

Required Qualifications

• Experience: * Minimum of 1 year of Certified Home Care experience (Required).
  • Minimum of 1 year of Case Management experience (Required).
• Travel: Ability and willingness to travel to cases assigned within the North Region (Chelsea, East Boston, Revere, and Winthrop).
• Clinical Skills: Proficiency in home health admission processes and electronic medical record (EMR) documentation.

While the supervisor’s office is located in Newton, MA, the primary field territory for this "Float" position covers the North Region, specifically:

• Chelsea
• East Boston
• Revere
• Winthrop
