Case Manager – Acute Care Hospital
Shift: Day | 5x8-hour shifts
Hours: 8:00 AM – 4:30 PM
Start Date: ASAP
Floating: Not required
Patient Ratio: 1:20
 

Position Summary
The Case Manager is responsible for providing comprehensive care coordination and discharge planning services for patients in an acute care hospital setting. This role collaborates closely with interdisciplinary teams to ensure safe, timely, and effective patient transitions while meeting hospital and regulatory requirements.

Key Responsibilities
Perform care coordination and discharge planning for hospitalized patients
Collaborate with physicians, nurses, social workers, and interdisciplinary care teams
Assess patient needs, barriers to discharge, and post-acute care requirements
Coordinate services, resources, and follow-up care to support safe discharge
Communicate effectively with patients and families regarding care plans
Document care coordination activities in the medical record
Ensure compliance with hospital policies, regulations, and standards of care

Required Qualifications
Minimum 2 years of acute care hospital Case Manager experienceMust have hands-on experience performing care coordination and discharge planning
Hospital experience is required
California State License (required)Pending license accepted; must be active by start date
First-time travelers accepted (clinical interview required)

Schedule Requirements
Weekend rotation required based on department needs
Request-off (RTO) limited to no more than 7 days
