Position Overview

We are seeking an experienced Hospital Case Manager (RN) to support care coordination and discharge planning within an acute care hospital setting. This role requires strong clinical judgment, excellent communication skills, and proven experience working as a case manager in a hospital environment.

The ideal candidate will be comfortable managing a high patient load and collaborating with interdisciplinary teams to ensure safe and effective patient transitions of care.

Key Responsibilities

• Perform care coordination and discharge planning for acute care patients
• Manage a caseload of approximately 20 patients
• Collaborate with physicians, nurses, social workers, and ancillary staff
• Assess patient needs, barriers to discharge, and appropriate level of care
• Coordinate post-acute services including home health, SNF, rehab, and DME
• Communicate with patients and families regarding discharge plans
• Document case management activities accurately and timely
• Ensure compliance with hospital policies and regulatory standards

Required Qualifications

• Active California RN License (pending license accepted, must be active by start)
• Minimum 2 years acute care Hospital Case Manager experience
• Strong background in care coordination & discharge planning
• Hospital experience is MANDATORY
• Ability to manage high-volume caseloads
• Strong communication and organizational skills

• Weekend: Weekend rotation required based on department needs
• Floating: No
• Patient Ratio: 1:20
• RTO: Maximum 7 days
• Scrubs: Scrubs or business attire
• Facility Type: Acute Care Hospital
