• Weekend Requirement: Every other weekend / dept needs
• Patient Ratio: 1:20
• Unit: Case Management (Acute Care Hospital)

Requirements

• CA RN license (pending accepted; must be active by start)
• Minimum 2 years acute care hospital Case Manager experience (care coordination & discharge planning required)
• Epic charting experience (required)
• Travel experience required (1st-time travelers accepted)
• Acute hospital experience only (no behavioral health–only background)

Key Skills

• Case management & utilization review
• InterQual criteria (preferred)
• Care coordination & discharge planning
• DRGs, CMS knowledge (preferred)
• Arrange DME, SNF, ambulance transport independently
• Experience with HMOs/IPAs; family meetings; complex social needs

Compliance / Submission Notes

• Skills checklist required with submission
• 2 supervisory references (within last 3 years)
• Interview availability, DOB & full SSN must be confirmed
• RTO ≤ 7 days preferred
• Must commit to 3 of 5 major holidays
• Local radius: 50 miles (locals accepted at lower bill rate)
• Floating within state required
