Title: Physical Therapist – Outpatient / Inpatient

Location: Ganado, AZ 86505

Duration: 13 weeks

Shift:

• Day Shift: 5x8s (08:00 – 16:00)
• Night Shift: 5x8s (21:00 – 09:00)
• Majority schedule: Monday–Friday 8:00 AM–5:00 PM (combination of inpatient AM/outpatient PM first week; remaining weeks primarily outpatient)

Job Responsibilities:

• Provide physical therapy services in both outpatient and inpatient settings.
• Perform patient evaluations and develop individualized treatment plans.
• Deliver therapeutic interventions and monitor patient progress.
• Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
• Maintain accurate and timely documentation in the electronic medical record system (Meditech).
• Ensure compliance with facility policies, regulatory standards, and patient confidentiality guidelines.
• Meet productivity and compliance requirements within established deadlines.

Required Skills & Qualifications:

• Minimum 2 years of Physical Therapy experience (outpatient and inpatient experience required).
• Valid State Physical Therapy License (must be active by start date).
• Experience with Meditech charting system.
• Copy of all required certifications.
• Ability to work in a government facility environment (no background hits accepted).
• COVID-19 vaccination required (no exemptions).
• Strong communication and organizational skills.
• Ability to adapt to a split inpatient/outpatient schedule during initial week.
