Job Overview

We are seeking an experienced RN Case Manager for a 13-week travel assignment in York, ME. This is a hospital-based care management role focused on utilization review, discharge planning, and interdisciplinary coordination.

Start Date: March 16, 2026 (ASAP consideration)
Contract Length: 13 weeks
Schedule Options:

  • 5x8 Hour Day Shifts (8:30 AM – 4:30 PM)
    OR
  • 4x10 Hour Day Shifts (7:30 AM – 5:00 PM)
    Hours: 40 hours per week

No call required. Weekend and holiday rotation required.

Responsibilities

  • Coordinate concurrent utilization management activities hospital-wide
  • Serve as the primary liaison between patient, healthcare team, organization, and payors
  • Conduct comprehensive patient assessments to identify discharge and transition needs
  • Develop and implement safe, cost-effective discharge plans
  • Secure post-acute resources including DME, transportation, and insurance authorizations
  • Collaborate and lead interdisciplinary team discussions during daily care rounds and conferences
  • Participate in length-of-stay management and quality improvement initiatives
  • Maintain thorough documentation within hospital EMR systems

Requirements

  • Active RN license (must be currently active)
  • BLS certification
  • Associate or Bachelor’s degree in Nursing
  • 3–5 years of clinical experience (Med/Surg, home care, hospital case management, and/or utilization review)
  • Must have utilization review and discharge planning experience
  • Strong written, verbal, and electronic communication skills
  • Working knowledge of hospital reimbursement methods and quality measures

Preferred Qualifications

  • CCM (Certified Case Manager) or ACM (Accredited Case Manager)
  • Certification through a nationally recognized case management or utilization review organization

Additional Details

  • No call requirement
  • Weekend and holiday rotation required
  • 54-bed hospital setting
  • Paid orientation (must complete 3 scheduled shifts)
  • 40 guaranteed hours per week
  • Timecards must be completed accurately to avoid processing delays
  • Credentials due 11 days prior to start (by noon)
Hospital-based RN Case Manager role requiring strong utilization review and discharge planning experience. Must be comfortable coordinating interdisciplinary care and working directly with payors and community resources. 40 hours guaranteed. No call, but weekend and holiday rotation required. CCM/ACM preferred.
