Description
A Case Manager RN in a hospital setting plays a critical role in coordinating patient care from admission through discharge, ensuring patients receive appropriate, cost-effective, and timely care.
Care Coordination
- Assess patients shortly after admission to identify medical, social, and discharge needs
- Develop and implement a care plan in collaboration with physicians, nurses, therapists, and social workers
- Coordinate services such as physical therapy, home health, durable medical equipment, or skilled nursing placement
Utilization Review & Insurance Management
- Review medical records to ensure the level of care (inpatient vs. observation) is appropriate
- Communicate with insurance companies for authorizations and continued stay approvals
- Prevent delays in care due to insurance or documentation issues
Discharge Planning
- Start discharge planning early in the hospital stay
- Arrange safe transitions to:
- Home with services
- Skilled nursing facilities
- Inpatient rehab
- Long-term acute care hospitals
- Ensure prescriptions, follow-up appointments, and patient education are in place before discharge
Patient & Family Advocacy
- Educate patients and families about diagnoses, treatment plans, and post-discharge care
- Address barriers such as transportation, financial concerns, or lack of caregiver support
- Advocate for appropriate resources and services
Documentation & Compliance
- Maintain detailed documentation to meet hospital, regulatory, and insurance standards
- Ensure compliance with Medicare/Medicaid and private payer guidelines
Requirements and Qualifications
- Active RN license
- BLS certification
- Minimum of one (1) year of experience
