Title: RN – Case Manager (UR/Discharge Planning)
Location: Santa Clara, CA
Position Type: 13 Weeks (40 Hours/Week – 520 Total Hours)
Shift: Days – 5x8 Hour Shifts (08:00 AM – 04:30 PM)

Job Description:

• Specialty: Case Management – Utilization Review & Discharge Planning
• Certifications: BLS required
• License: Active CA RN License required
• Schedule: Monday–Friday, 8-hour day shifts
• Setting: Hospital-based UR/Discharge Planning department
• Responsible for coordinating patient care, discharge planning, utilization review, and collaboration with interdisciplinary teams to ensure appropriate level of care and resource management
