Description

M–F 8:00a–4:30p Role: Pediatric RN Case Manager providing in-home hospice and concurrent care services for patients birth–21 years. Responsible for care coordination, skilled nursing assessments, symptom management, and family education. 4–5 home visits per day. Details: On-call approx. 7 days/month 1:15 caseload ratio Charting: NetSmart Requirements: Active CA RN License Minimum 2 years pediatric experience (PICU, NICU, pediatric acute care, oncology, and/or hospice) Hospice or case management experience preferred BLS required Reliable transportation Valid driver’s license Current auto insurance with acceptable coverage Current vehicle registration. Confirm candidate is comfortable with home visits and on-call rotation
