Summary:

Role: Case Manager RN
Schedule: 5x8s, every other weekend
Requirements:
- Active CA RN License
- BLS certification
- Minimum 5+ years of RN Case Management experience
Key Responsibilities:
- Coordinate patient care and manage care transitions across the acute care continuum
- Collaborate with physicians, social workers, nurses, and interdisciplinary teams
- Conduct patient assessments (medical, psychosocial, financial) and identify readmission risks
- Develop and implement cost-effective care coordination and discharge plans
- Monitor level of care (LOC), length of stay (LOS), and ensure proper documentation
- Facilitate timely transitions to appropriate levels of care and prevent unnecessary admissions/readmissions
- Address complex clinical and social situations, including discharge barriers
- Ensure compliance with CMS, Joint Commission, and regulatory requirements
- Communicate effectively with patients, families, payers, and community resources
