Description

Shift: Monday-Friday 8a-4:30p

Weekend Requirement: One weekend per month required

Patient Population

Responsible for clinical assessment, patient care planning, teaching, and discharge requirements

Required Certifications
Must be attached at submission:

RN required

Minimum Experience Requirements

Min. one year acute care hospital experience required - will consider IRF, LTAC

Preferred:

3 Years discharge planning preferred
Mother/baby experience highly preferred

Required Skills

Screening for care level, discharge planning, chart review, and patient education

Preferred Skills

EPIC, CarePort Parachute experience
