Travel RN – Case Manager (Day Shift)

Position Details

Role: Registered Nurse (RN) – Case Manager
Schedule: 3x12-hour shifts (36 hours/week)
Shift: Day shift with weekend rotation
Duration: 13 weeks
Start Date: ASAP
On-Call: None

Key Responsibilities

Coordinate and manage patient care plans across the continuum
Support both inpatient and outpatient case management needs
Collaborate with interdisciplinary teams to ensure optimal patient outcomes
Assist patients and families with care planning, discharge needs, and resource coordination

Requirements

Minimum 3 years of RN Case Management experience (hospital setting required)
Epic EMR experience – REQUIRED
Active RN license (state of Colorado or compact)
Graduate of an accredited nursing program
Flu vaccine required (no exemptions)

Must live 75+ miles away from the facility to qualify as a traveler
