Travel RN – Case Manager
📍 Santa Rosa, CA
🗓 Start: 04/20/2026 | Duration: 13 weeks
🕒 Schedule: Day Shift 5x8 (08:00 – 16:30)
🔎 Requirements:
• 2+ years Acute Hospital Case Management experience
• Strong experience in Care Coordination & Discharge Planning (fast-paced setting)
• Epic charting required
• Travel experience preferred (can be considered)
• BLS required
• Recent experience within last 2 years (HARD STOP)
💡 Key Responsibilities:
• Care coordination & discharge planning
• Utilization review & medical necessity (InterQual preferred)
• Arrange DME, SNF placement & transport independently
• Participate in treatment plans & family meetings
