Title: Inpatient RN Case Manager

Duration: 12+ Weeks + High possibility of extension

Location: Los Angelos/Inglewood, CA

Schedule Notes: 8-hour shifts (8:00 AM – 4:30 PM)

Job Summary:

We are seeking an experienced Inpatient Case Manager to join our team for a 12-week assignment. The ideal candidate will have strong inpatient case management experience, with the ability to coordinate patient care, ensure appropriate utilization of resources, and facilitate safe and timely discharge planning.

Key Responsibilities:

• Manage and coordinate care for patients in an inpatient setting
• Conduct initial and ongoing patient assessments
• Develop and implement individualized care and discharge plans
• Collaborate with physicians, nurses, and multidisciplinary teams
• Perform utilization review to ensure medical necessity and appropriate level of care
• Facilitate communication between patients, families, and healthcare providers
• Arrange post-discharge services such as home health, rehab, or SNF placement
• Ensure compliance with hospital policies, regulatory guidelines, and payer requirements
• Document all case management activities accurately and timely 

Requirements:

• Must have recent inpatient case management experience
• Active RN license
• Strong knowledge of utilization review and discharge planning
• Experience working in acute care hospital settings
• Excellent communication and organizational skills
• Ability to work independently and in a fast-paced environment
• Flexibility to float to Kaiser Centinela location as needed
