Job Summary

We are seeking an experienced Registered Nurse – Case Manager to join an acute care setting at Providence Mission Hospital in Mission Viejo, CA. The Case Manager will be responsible for providing comprehensive care coordination, discharge planning, and utilization of healthcare resources to ensure optimal patient outcomes across the continuum of care.

This role requires strong travel experience (HARDSTOP) and at least 2 years of acute medical case management experience focused on care coordination.

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans
• Coordinate discharge planning and post-acute care services
• Collaborate with interdisciplinary teams to ensure continuity of care
• Arrange DME, SNF placements, and ambulance transport independently
• Facilitate family meetings and participate in treatment plan development
• Provide care coordination for complex patient populations
• Handle cases involving mental health, substance abuse, homelessness, foster/adoption, and protective services
• Work with HMOs, IPAs, CMS, DRG guidelines, and regulatory standards
• Ensure compliance with HIPAA, OSHA, Joint Commission, and CMS regulations
• Utilize Epic EMR system for all documentation (required)

Required Experience & Qualifications

• Minimum 2 years of acute care Case Management / Care Coordination experience
• Travel nursing experience (required – HARDSTOP)
• Experience in acute hospital setting
• Familiarity with Emergency Department, ICU, Medical-Surgical, or Surgical Services (preferred)
• Strong experience in discharge planning and utilization coordination
• Knowledge of regulatory standards (CMS, Joint Commission, OSHA, HIPAA)
• Ability to work independently in a fast-paced hospital environment

Schedule & Work Expectations

• Day shift: 8:00 AM – 4:30 PM
• Weekend rotation required (every other weekend)
• Floating required to Laguna facility
