Description
Position: Hospital at Home RN
Shift: Rotating 12-Hour Shifts
- 7:00 AM – 7:00 PM
- 11:00 AM – 11:00 PM
Position Overview:
The Hospital at Home RN provides acute-level inpatient care to patients in their home environment. This role delivers hospital-level care through in-home visits, remote monitoring, and coordinated multidisciplinary support, ensuring high-quality and safe patient outcomes.
The RN is responsible for implementing individualized care plans, performing assessments, and delivering therapeutic interventions using a team-based nursing model. This role integrates technology and remote clinical coordination to replicate inpatient care delivery within the patient’s home.
Responsibilities:
- Provide direct patient care in the home setting at an acute/inpatient level
- Perform assessments, interventions, and documentation using the nursing process
- Collaborate with a multidisciplinary care team
- Utilize remote monitoring tools and healthcare technology
- Ensure compliance with safety, quality, and care standards
Requirements:
- Minimum 3 years of RN experience in critical care, acute care, or medical-surgical
- BSN required
- Active VA or Compact RN license at time of submission
