Unit Overview:

• Number of Beds: 350
• Staff Composition: Case Managers (CM), Social Workers (SW), and SNF Coordinator
• Shift Coverage:
  • Day Shift: As per unit structure (e.g., 1 charge, 3 RN, 2 CNA, 1–3 sitters)
  • Night Shift: N/A
• Patient Ratio: 25:1
• Equipment: N/A
• EMR Systems: Meditech and Midas

Key Requirements (Must Have):

• Proven acute care case management (CM) experience
• Ability to manage a patient ratio of 25:1 in a fast-paced environment
• Strong understanding of case management workflows, discharge planning, and care coordination
• Ability to work independently while collaborating with interdisciplinary teams
• Strong communication, organizational, and time management skills

Preferred Qualifications:

• Experience with Meditech and Midas documentation systems
• Prior HCA facility experience

• Accurate and timely documentation
• Collaboration with CM, SW, and SNF coordination teams
• Ability to prioritize and manage multiple cases efficiently

Scheduling & Work Expectations:

• Shift: 8:00 AM – 5:00 PM
• Travelers will be scheduled in accordance with facility policy
• Weekend Requirement: Every other weekend
• Holiday Requirement: Rotational, in alignment with hospital staff
