Job Overview
We are seeking an experienced Case Management Registered Nurse to support a fast-paced acute care environment. The RN will focus on care coordination, discharge planning, and utilization management for adult and geriatric patients.

Schedule & Assignment Details

• Shift: Day shift (5x8s | 8:00 AM – 4:30 PM)
• Contract Length: 13 weeks
• Weekend Requirement: 2 weekends every 6 weeks (or per department needs)
• Floating: Yes, based on facility needs
• Call: Not required
• Locals: Accepted (at a lower pay rate)

Required Experience

• Minimum 2 years of acute hospital Case Management experience
• Experience in care coordination and discharge planning in a fast-paced environment
• Recent relevant work history within the last 2 years (HARD STOP) 

Required Certifications

• BLS from AHA (Basic Life Support) 

Key Responsibilities

• Perform comprehensive discharge planning and care coordination
• Assess patient needs and determine appropriate level of care
• Collaborate with interdisciplinary teams for treatment planning
• Arrange DME, SNF placement, home health, and transportation services independently
• Participate in family meetings and care conferences
• Ensure compliance with payer guidelines (CMS, DRG, InterQual, etc.)
• Support utilization review and medical necessity determinations

• Full SSN is required at the time of submission
• Local candidates will be considered at a lower pay rate

Required Skills & Experience

• Acute hospital case management
• Discharge planning & care coordination
• Utilization review (InterQual preferred)
• Knowledge of CMS guidelines and regulatory standards
• Experience with DME, SNF, and ambulance coordination
• Familiarity with Epic EMR
