Job Description
We are seeking an experienced Wound Care Registered Nurse (RN) to join the team at Harlem Hospital Center. The RN will specialize in wound, ostomy, and continence care, providing comprehensive patient assessments, treatment planning, and education.
Key Responsibilities
- Perform focused assessments for wound, ostomy, and continence care
- Develop and implement individualized care plans
- Monitor and document patient progress and response to treatment
- Collaborate with interdisciplinary healthcare teams
- Provide patient and family education on wound management and prevention
- Conduct staff training and participate in continuing education programs
- Identify patients at risk for pressure injuries and initiate preventive care
- Participate in quality improvement initiatives and hospital programs
Required Qualifications
Experience:
Minimum 2 years of Med/Surg experience
Wound care and ostomy experience required
Education:
Associate Degree in Nursing (ASN/ADN) from an accredited program
Licensure & Certifications:
Active New York RN License
AHA BLS Certification
One of the following certifications required:
CWOCN
CWON
CWCN
CWS
WCC
