Description
RN Home Health Case Manager
Patient Types/Common Diagnoses: Wound care, COPD, CHF, Diabetes, total joints, Comp assess, medication management, IV medications, picc line
o
● Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
● Registered Nurse license issued by the state in which the team member practices, and
● Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required, and
● A valid drivers license issued by the Division of Motor Vehicles
