Requirements – Case Manager (Inpatient Discharge Planning)

Experience

• Minimum 1 year of inpatient Case Management experience (required)
• Experience with discharge planning in acute care settings
• Critical care experience preferred
• Ability to manage a caseload of 20+ patients independently

Licensure & Certifications

• BLS (Basic Life Support) – Required
• PRI & SCREEN Certifications – Required (can be completed online via School of Albany; approx. $300 each)

Core Skills & Competencies

• Strong knowledge of discharge planning and care coordination
• Ability to work independently on assigned units
• Experience collaborating with an interdisciplinary team (physicians, social workers, etc.)
• Strong critical thinking, time management, and communication skills
• Ability to round with physicians and coordinate patient care plans

Technical Skills

• Experience with EMR systems (Allscripts preferred)
• Familiarity with CarePort, MCG, and/or InterQual (preferred)
• Basic understanding of Utilization Review (UR) processes (preferred, not required)

Work Environment & Responsibilities

• Focus on inpatient discharge planning (not UR)
• Manage patients across Med-Surg and Telemetry units
• Coordinate discharges to:
  • Home / Community
  • Skilled Nursing Facilities (SNF)
  • Special cases (homeless shelters, dialysis coordination, wound vac referrals, etc.)

Work Requirements

• Must be able to handle dynamic caseloads (12–25 patients depending on unit)
• Willingness to occasionally work weekends/holidays (rare, with approval)
• Ability to cover multiple units when needed

Additional Expectations

• Maintain a positive attitude and team-oriented approach
• Ability to work in a fast-paced hospital environment
• Participate in orientation (2 days hospital + 1–2 weeks preceptorship)
