Job Summary:

We are seeking a Clinical Pharmacist Coordinator to oversee pharmacy clinical services (CPS) and support clinical operations. This role is responsible for guiding clinical decision-making, managing patient care programs, and leading quality improvement initiatives to ensure optimal medication use and patient outcomes.

Shift Details:

• Day Shift

Key Responsibilities:

• Provide oversight of Pharmacy Clinical Services (CPS)
• Guide clinical operations and support decision-making for pharmacists
• Develop, implement, and maintain patient management programs
• Ensure policies and procedures are up to date and compliant
• Support and advise clinical pharmacists on patient care decisions
• Lead and implement Quality Improvement Programs (QIP)
• Monitor medication use systems and improve patient outcomes
• Collect and analyze data; report findings to quality committees
• Oversee training and development of clinical pharmacy staff
• Provide direct patient care when needed
• Collaborate with physicians, nurses, and healthcare teams

Required Qualifications:

• Graduate of an accredited School of Pharmacy
• Active Pharmacist License (State-specific)
• Strong knowledge of clinical pharmacy practices and patient care

Preferred Qualifications:

• 2+ years of Pharmacist experience
• 2+ years of pharmacy leadership experience
• Experience in clinical program management and quality improvement
• Strong leadership, communication, and analytical skills

Work Setting:

• Hospital / Clinical Pharmacy Department
• Leadership and patient-facing responsibilities
