Description
Role: Care Manager RN
Duration: 13 weeks
Schedule & Hours:
- Variable shift (not fixed)
- Every other weekend required
- No on-call
- No block scheduling
- 48 hours not approved
Patient Load:
- Ratio: 20:1
Requirements:
- Minimum 3 years inpatient care management experience
- Recent acute care hospital CM/discharge planning experience required (behavioral health experience does NOT count)
- Must be flexible with scheduling (to support PTO/vacation coverage)
- Must have a personal vehicle
Floating:
- Yes, required based on unit needs
- May float across multiple facilities: Milwaukie, Newberg, Portland, Hood River, Willamette Falls, Seaside, Saint Vincent
- Hotel provided for consecutive shifts if floating to Seaside
