Description
- Minimum 2 years acute hospital Case Management experience (recent within last year)
- Must have experience in care coordination & discharge planning
- Epic charting experience required
- Experience with HMOs, IPAs, advance directives, POLST, and regulatory guidelines
- Travel experience required (1st timers may be considered after screening)
- Must be willing to float and work every other weekend
