 Job Overview

We are seeking an experienced Registered Nurse (RN) with a strong background in Med-Surg/Telemetry and charge nurse responsibilities, along with exposure to discharge planning and care coordination. This role focuses on enhancing patient care delivery, supporting clinical teams, and ensuring smooth care transitions.

  Key Responsibilities

• Provide high-quality nursing care to Med-Surg/Telemetry patients
• Perform proactive patient rounding to assess needs and improve patient experience
• Support and collaborate with bedside nurses and interdisciplinary teams
• Utilize charge nurse experience to assist with workflow coordination and patient flow
• Participate in discharge planning and care management activities
• Identify and escalate clinical concerns appropriately
• Promote patient safety, fall prevention, and quality care initiatives
• Document all care activities in EMR (EPIC preferred)

  Requirements

• Active California RN License
• Minimum 2+ years Med-Surg/Telemetry experience
• Charge Nurse experience required
• Experience or exposure to discharge planning / care coordination
• BLS Certification (AHA)
• Strong communication and organizational skills

  Preferred Qualifications

• Previous experience in patient flow, care coordination, or resource nurse roles
• Familiarity with EPIC EMR
• Background in improving patient satisfaction and safety outcomes
