Description
Job Summary:
We are seeking an experienced RN Case Manager with a strong background in acute care hospital settings. This role focuses on care coordination, discharge planning, and utilization review in a fast-paced environment. The ideal candidate will ensure efficient patient transitions while maintaining compliance with regulatory standards.
Key Responsibilities:
- Perform care coordination and discharge planning for hospitalized patients
- Conduct needs assessments and develop individualized care plans
- Handle admission criteria, benefits eligibility, and prior authorizations
- Manage appeals and denials when applicable
- Coordinate with interdisciplinary teams to ensure optimal patient outcomes
- Arrange DME, post-acute care services, and follow-ups
- Maintain accurate documentation in Epic EMR
- Ensure compliance with CMS, HIPAA, Joint Commission, and DRG guidelines
- Work across multiple units as needed (floating required)
Requirements (Must Have):
- Minimum 2 years of acute hospital Case Management experience (MANDATORY)
- Strong experience in Care Coordination & Discharge Planning
- Acute care experience is required (non-negotiable)
- Experience with Epic EMR
- Ability to work in a fast-paced hospital environment
- Travel experience required (1st time travelers can be screened)
Preferred Qualifications:
- Experience in ICU, ED, Med-Surg, Pediatrics, or Step-down units
- Knowledge of prior authorizations, appeals/denials, and utilization review
- Familiarity with regulatory standards (CMS, DRG, Joint Commission)
Work Details:
- Weekend Requirement: Every Other Weekend (EOW)
- On-call: Not required
- Floating: Yes, based on unit needs
- Locals accepted: Yes
- Charting System: Epic
- Parking: Free (first day), assignment parking provided after orientation
