Description
Position Overview
We are seeking an experienced and compassionate Hospice Registered Nurse to provide high‑quality end‑of‑life care to patients and families across multiple parishes in the Baton Rouge area. This role focuses on admissions, recertifications, interdisciplinary group documentation, and routine hospice visits while ensuring compliance with hospice philosophy, physician orders, and regulatory standards.
Schedule & Work Details
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 36 hours
- Productivity Expectation: 40 points per week
- Coverage Area: East Baton Rouge, West Baton Rouge, Iberville, Assumption, and Ascension Parishes
- On‑Call/Weekend Requirement: Every fifth weekend
- Mileage Reimbursement: Yes, reimbursed between patient visits
- Pay Rate: $51 to $53 per hour
Key Responsibilities
- Perform hospice admissions, recertifications, and routine day‑to‑day skilled nursing visits
- Complete accurate and timely Interdisciplinary Group (IDG) documentation and participate in care planning
- Conduct comprehensive patient assessments and manage individualized hospice plans of care
- Provide symptom management, patient and family education, and emotional support consistent with hospice philosophy
- Communicate changes in patient condition promptly to physicians and interdisciplinary team members
- Maintain accurate, compliant documentation in the electronic medical record system
- Collaborate closely with hospice team members to ensure continuity and quality of care
- Adhere to agency policies, infection control standards, and state, federal, and CMS hospice regulations
Required Qualifications
- Active Registered Nurse license
- Associate Degree in Nursing (ASN)
- Prior hospice nursing experience
- Strong clinical assessment, documentation, and communication skills
- Ability to work independently in a field‑based hospice role
Preferred Qualifications
- Experience using Homecare HomeBase (HCHB)
- Strong understanding of hospice regulations, documentation standards, and symptom management
