Description

Position Overview:
We are seeking a compassionate and experienced Hospice Registered Nurse to provide high-quality, patient-centered care to individuals in a home setting within the Lafayette area. This role focuses on delivering end-of-life care with dignity, supporting patients and families, and collaborating with an interdisciplinary team to ensure optimal comfort and outcomes.

Schedule & Work Details:

  • Location: Lafayette and surrounding areas within a 50-mile radius
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Guarantee: 40 hours
  • On-Call/Weekend Requirement:
    • 1 Friday every 8 weeks
    • 1 Saturday and Sunday every 4 weeks
  • Mileage Reimbursement: Yes, between patient visits (first to last patient)
  • Pay Rate: $51 to $53 per hour

Key Responsibilities:

  • Provide skilled nursing care to hospice patients in their homes
  • Perform comprehensive patient assessments and monitor for changes in condition
  • Develop, implement, and update individualized plan of care in collaboration with the interdisciplinary team
  • Provide symptom and pain management to ensure patient comfort
  • Educate patients and families on disease progression, medications, and end-of-life care
  • Coordinate care with physicians, social workers, chaplains, and other care team members
  • Respond to on-call needs and provide support during scheduled rotation
  • Maintain accurate, timely, and compliant clinical documentation
  • Support patients and families emotionally during the hospice journey

Required Qualifications:

  • Active Registered Nurse (RN) license
  • Current BLS (Basic Life Support) certification
  • Prior hospice nursing experience required
  • Strong clinical assessment and critical thinking skills
  • Ability to work independently in a field setting

  • Positive, flexible, and team-oriented attitude
  • Strong communication and interpersonal skills
  • Ability to adapt to changing patient needs and schedules
