Position Overview

Ochsner Home Health of Raceland is seeking an experienced Home Health Registered Nurse to provide high quality skilled nursing care to patients in their homes. This role involves delivering comprehensive patient care in the field, performing advanced clinical skills, and managing a diverse patient population while meeting productivity expectations. The RN will work independently while collaborating closely with the interdisciplinary care team.

Schedule & Work Details

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hour Guarantee: 40 hours
• Productivity Expectations: 30 points
• On-Call/Weekend Requirement: Not required, but willingness to assist if needed due to call-ins
• Coverage Area: 50 mile radius from the Raceland office
• Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient
• Pay Rate: $51–$53 per hour

Key Responsibilities

• Provide direct skilled nursing care to home health patients in the field
• Perform and manage a wide range of advanced clinical skills, including IVs, PICC lines, midlines, JP drains, tracheostomy care, lary tubes, and Pleurx drains
• Complete indwelling and suprapubic catheter changes
• Manage wound vacs and provide comprehensive wound care
• Perform PEG tube and ostomy care
• Conduct thorough patient assessments and implement individualized plans of care
• Educate patients and caregivers on treatment plans, disease processes, and proper self-care techniques
• Accurately document all patient care and visits in accordance with agency and regulatory standards
• Communicate effectively with physicians, case managers, and interdisciplinary team members

Required Qualifications

• Active Registered Nurse (RN) license
• Prior home health nursing experience
• Ability to safely and independently perform advanced skilled nursing procedures
• Strong clinical assessment and documentation skills
