Job Summary:

Seeking an experienced Occupational Therapist for Home Health skilled visits treating adult med/surg, cardiac, orthopedic, neuro, respiratory, and post-surgical patients. Therapist will provide independent in-home therapy services, patient education, mobility training, ADL support, and documentation for skilled home care visits.

Key Responsibilities:

• Perform skilled occupational therapy home visits (6–8 visits per day)
• Provide ADL training, mobility training, safety and injury prevention education
• Develop Home Exercise Programs (HEP) and educate patients/families
• Treat patients with orthopedic, neuro, cardiac, respiratory, trauma, and post-surgical conditions
• Complete documentation and billing accurately using BayCare documentation systems

Required Skills:

• Assessment and documentation skills
• Cardiopulmonary/circulatory care knowledge
• Experience with mobility devices and adaptive equipment
• Orthopedic and neurological rehabilitation experience
• Patient/family education and safety training
• Medication and symptom monitoring

Patient Population:

• Med/Surg
• Cardiac and respiratory patients
• COPD, CHF, Pneumonia
• CVA, Parkinson’s, weakness, falls
• Trauma, AKA/BKA, neuro patients
• Hip/Knee/Shoulder replacements
• Post-surgical and ADL support patients

Requirements:

• Active State Occupational Therapist (OT) License
• Active BLS Certification
• Minimum 1 year of Home Health experience required
• OASIS experience/training preferred
• Valid driver’s license and reliable vehicle required
• Must be able to work independently in home settings
• COVID vaccination required (or approved exemption)
• Flu shot required or signed declination with mandatory mask use
