Description
Position Overview:
Seeking a compassionate and experienced Registered Nurse to provide high-quality, patient-centered hospice care in a home-based setting. This role focuses on supporting patients and families through end-of-life care by delivering comprehensive nursing services, managing symptoms, and coordinating with the interdisciplinary care team to ensure comfort, dignity, and quality of life.
Schedule & Work Details:
- Location: Field-based role serving Northwest Arkansas (Sebastian, Logan, Crawford, Franklin, Scott, and Yell counties)
- Schedule: Monday through Friday, 8:00 AM to 5:00 PM
- Weekly Hour Guarantee: 32 hours
- On-Call Requirements: Participation in backup on-call rotation
- Mileage Reimbursement: Per agency policy
- Pay Rate: $51 to $53 per hour
Key Responsibilities:
- Provide skilled hospice nursing care in accordance with physician orders and individualized plans of care
- Perform comprehensive patient assessments and ongoing evaluations to monitor symptom management and disease progression
- Develop, implement, and update care plans in collaboration with physicians and the interdisciplinary team
- Manage pain and symptoms to ensure patient comfort and quality of life
- Provide emotional support and education to patients and families regarding disease progression, medications, and end-of-life care expectations
- Coordinate care with hospice team members, including social workers, chaplains, and aides
- Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory and agency standards
- Communicate effectively with physicians, patients, caregivers, and team members regarding changes in condition and care needs
- Respond to patient needs and participate in backup on-call rotation as required
- Support patient and family through the dying process with compassion and professionalism
Required Qualifications:
- Active Registered Nurse (RN) license in the applicable state
- Ability to provide compassionate end-of-life care in a home setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently in a field-based environment
- Reliable transportation and ability to travel throughout assigned coverage area
Preferred Qualifications:
- Previous hospice or home health experience
- Experience with symptom management, pain control, and end-of-life care
- Strong organizational and time management skills
- Ability to work collaboratively within an interdisciplinary team
