Description
Responsibilities
- Evaluate and treat patients in their homes following illness, injury, surgery, or hospitalization
- Develop individualized treatment plans focused on mobility, strength, balance, and functional independence
- Complete Start of Care, evaluations, reassessments, and discharge visits
- Collaborate with nursing, OT, SLP, physicians, and interdisciplinary care teams
- Educate patients and caregivers on exercises, safety, mobility, and home programs
- Maintain accurate EMR documentation and comply with home health regulations
Qualifications
- Graduate of an accredited Physical Therapy program
- Active Nevada PT license or ability to obtain one
- Home Health experience preferred
- Oasis documentation experience preferred
- Strong time management and independent clinical decision-making skills
- Valid driver’s license and reliable transportation required
