The RN Clinical Resource/Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

Minimum Requirements:

Locals accepted at $88/hr!

• Bachelor Degree Required. Master's Degree preferred.
• 5 Years of Case Management in acute care setting within past 5 years
• 2 Years experience in process improvement
• Epic experience required
• Knowledge of CMS regulations
• Management/Supervisory experience preferred
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
