RN – Case Manager Travel Assignment | Apple Valley, CA

This role focuses on care coordination, discharge planning, utilization review, and ensuring appropriate transitions of care.

Schedule & Contract Details:

• Hours: 8:00 AM – 4:30 PM
• Every Other Weekend Rotation

Requirements:

• Active California RN License
• Minimum 2 years of Acute Care Case Management experience
• Minimum 2 years of Discharge Planning experience
• Epic experience required
• Previous travel experience required
• First-time travelers not accepted
• Hospital-based Case Management experience required
• Behavioral Health-only experience does not qualify

Responsibilities Include:

• Patient assessment and care coordination
• Discharge planning and utilization review
• Participation in interdisciplinary and family meetings
• Development and implementation of treatment plans
• Coordination of DME, SNF placement, and transportation services
• Prior authorizations and payer communication
• Collaboration with physicians, nursing staff, social services, and therapy teams

Required Experience:

• Acute Care Hospital Case Management
• Utilization Review
• Care Coordination
• Discharge Planning
• CMS and Joint Commission compliance
• Experience with HMOs, IPAs, and insurance processes
• Knowledge of community resources and post-acute care services

• Average Caseload: Up to 25 Patients

• Patient Population: Neonatal, Pediatric, Adult, and Geriatric
