Acute Care Coordinator (Case Management)

Agency: Medical Solutions Location: Sacramento, CA Contract Length: 13 Weeks Shift: 8-Hour Days (Every Other Weekend Required) Guaranteed Hours: 40 hours per week

Role Overview

We are seeking an experienced and adaptable Acute Care Coordinator to join our team in an underserved Sacramento facility. You will manage complex, socially-impacted discharges and utilization review in an acute care setting. This role requires a professional who can hit the ground running, navigate challenging discharge barriers, and maintain excellent attendance.

Key Responsibilities

  • Case Management: Manage acute-level discharge planning, including R&B (Room and Board), B&C placements, and conservatorship cases.
  • Utilization Review: Apply strong knowledge of Medicare/Medi-Cal guidelines to facilitate appropriate patient flow.
  • Clinical Collaboration: Coordinate with placement agencies and manage discharges for uninsured or under-insured populations.
  • Floating: Must be willing to float to all adult hospital units (Women’s Health experience preferred but not required).

Required Qualifications

  • Experience: Minimum 3 years of hospital experience in an acute care setting.
  • Mandatory Requirement: Must have prior CA Dignity/CommonSpirit acute care experience.
  • Technical Proficiency: High proficiency in Midas, InterCall, Cerner, Curaspan, and Microsoft Office.
  • Compliance: Must have two supervisor references (charge nurse accepted); one from within the last 12 months, one from within the last 3 years.
  • Licensure: Valid Driver’s License required at time of submittal.
  • Certifications: BLS Certification required.
  • Stability: 40-hour guaranteed work week.
