We are seeking an experienced RN Case Manager to join a leading Acute Care Hospital. 
 

This role requires close collaboration with physicians, nursing staff, social workers, and other healthcare professionals to facilitate safe, efficient, and patient-centered transitions of care.

Minimum Qualifications

• Active California Registered Nurse (RN) License
• Minimum of 2 years of recent Acute Care Hospital Case Management experience
• Recent experience performing:
  • Care Coordination
  • Discharge Planning
  • Utilization of community and post-acute resources
• Acute Care Hospital Case Management experience is required
• Strong communication, organizational, and critical-thinking skills

Traveler Requirements

Key Responsibilities

• Coordinate patient care plans and discharge needs throughout hospitalization.
• Conduct comprehensive assessments to identify barriers to discharge and continuity of care.
• Collaborate with physicians, nurses, social workers, and interdisciplinary teams to develop effective care plans.
• Facilitate timely discharges and appropriate transitions to post-acute care settings.
• Educate patients and families regarding treatment plans, resources, and discharge instructions.
• Advocate for patients while ensuring quality, cost-effective care delivery.
• Maintain accurate and timely clinical documentation.
• Support compliance with hospital policies, regulatory requirements, and quality standards.
