Position Details

• Position: Home Health RN
• Shift: Day Shift | 5x8-Hour (8:00 AM – 4:30 PM)
• Schedule: Monday–Friday, Every 3rd Weekend
• Contract Length: 13 Weeks
• Charting System: EPIC
• Average Caseload: 6 points per day / 30 points per week
• Locals Accepted: Yes, at the same rate

Schedule & Requirements

• Monday–Friday schedule with every third weekend required
• Floating may be required based on agency needs
• Active Driver's License required
• Must be able to meet productivity expectations (30 points/week)
• RTO must be submitted with the initial application

Required Experience

• Minimum 2 years of RN experience
• Minimum 1 recent year of Home Health experience (required)
• Must have OASIS documentation experience
• Specialty experience must be within the last year
• Experience with Start of Care (SOC), Recertifications, Resumption of Care (ROC), and Home Health visits preferred

Required Certifications

• BLS Certification
• Active VA RN License or Compact RN License
• BLS must be through the American Heart Association (AHA)

Key Responsibilities

• Conduct home health nursing visits for adult and geriatric patients
• Complete OASIS assessments and documentation
• Perform Start of Care (SOC), Follow-up Visits, Recertifications, and Resumption of Care (ROC)
• Develop and implement patient care plans
• Coordinate care with Physical Therapy and Social Services
• Monitor patient conditions and provide education to patients and families
• Maintain accurate documentation in EPIC
