Home Health PT Opportunity – Yuma, AZ

Ardor Health Solutions is seeking a Physical Therapist for a 13-week Home Health assignment in Yuma, Arizona.

This is a Monday–Friday, 8–5 position with no weekends and no on-call, offering the independence of Home Health while providing one-on-one care to patients throughout the Yuma area.

Qualifications Required

  • Graduate of an accredited Physical Therapy program
  • Current Arizona PT license or Compact privilege
  • Current BLS certification
  • Previous Home Health experience preferred
  • OASIS experience preferred
  • Valid driver's license and reliable transportation
  • Strong evaluation, documentation, and time-management skills
  • Comfortable independently managing a Home Health caseload and territory

What You'll Do

  • Complete PT evaluations and reassessments in patients' homes
  • Develop individualized plans of care
  • Provide gait, balance, strengthening, transfer, and mobility training
  • Treat orthopedic, neurological, post-surgical, and medically complex patients
  • Provide fall-prevention and home-safety education
  • Educate patients and caregivers on home exercise programs
  • Collaborate with the interdisciplinary Home Health team
  • Complete timely clinical documentation

The position covers the Yuma area, with approximately 150–200 miles of travel per week.
