Description
Patient Care Coordinator (PCC)
Responsible for coordinating care and discharge planning for acute inpatient patients. Collaborates with physicians, patients, families, nursing staff, and community providers to ensure safe, cost-effective transitions of care and reduce readmissions.
Key Responsibilities:
- Complete patient assessments within 24 hours of admission.
- Develop and document discharge plans.
- Coordinate post-acute services, including Home Health, DME, IV Infusion, SNF, and Rehab.
- Communicate care plans with healthcare teams and patients/families.
Requirements:
- Associate Degree in Nursing.
- Minimum 1 year of acute care, Med/Surg, ED, or Critical Care nursing experience.
- Experience with EMR systems.
- Knowledge of Medicare, Medicaid, commercial payers, and CMS regulations.
- Completion of PCC Assessment of Critical Skills.

Staff constantly switched. My documents weren't sent to the school, resulting in a months lost pay. It is IMPOSSIBLE to get on touch with anyone to answer questions. I have been attempting to reach them for over a month with absolutely no response! They lose paperwork (tests, immunizations, background checks and fingerprints) resulting in even more lost work.
