Description
Patient Care Coordinator (PCC)
Responsible for coordinating care and discharge planning for acute inpatient patients. Collaborates with physicians, patients, families, nursing staff, and community providers to ensure safe, cost-effective transitions of care and reduce readmissions.
Key Responsibilities:
Complete patient assessments within 24 hours of admission.
Develop and document discharge plans.
Coordinate post-acute services, including Home Health, DME, IV Infusion, SNF, and Rehab.
Communicate care plans with healthcare teams and patients/families.
Requirements:
Associate Degree in Nursing.
Minimum 1 year of acute care, Med/Surg, ED, or Critical Care nursing experience.
Experience with EMR systems.
Knowledge of Medicare, Medicaid, commercial payers, and CMS regulations.
Completion of PCC Assessment of Critical Skills.

Staff constantly switched. My documents weren't sent to the school, resulting in a months lost pay. It is IMPOSSIBLE to get on touch with anyone to answer questions. I have been attempting to reach them for over a month with absolutely no response! They lose paperwork (tests, immunizations, background checks and fingerprints) resulting in even more lost work.
