Description

RN: Case Manager Division: Hospice Required to see patients daily and be able to help with on-call rotation. On-Call schedule is released 30 in advanced. Shift and work schedule: Monday thru Friday 8:00 am to 4:30 pm and participate in the on-call rotation Having many sites per day/visit (if applicable): approximately 21-25 visits per week Mileage reimbursement: $0.41 Wound Care Experience Required-No certification, just basic knowledge and skills HomeCare HomeBase (HCHB) & Hospice experience--Required Counties Covering: Coffee, Franklin, Grundy, and VanBuren CPR/BLS License Valid DL and Insurance Primary Responsibilities: Perform comprehensive assessment of high risk members
