Description
**Title:** Licensed Practical Nurse – Home Health
**Schedule:** 8 hours per shift
**Assignment Details:**
- Contract Length: 14 weeks
- Guaranteed Hours: 40 hours per week
**Requirements:**
- Current Nursing License in the practicing state
- Valid Driver's License
- Current CPR Certification
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
- Ability to use electronic medical charting and time-keeping software
**Roles & Responsibilities:**
- Provide skilled nursing care to patients in their residence as prescribed by physicians and in accordance with the established plan of care.
- Assist in identifying patient's physical, psycho-social, and environmental needs, and report outcomes to the supervising RN case manager.
- Participate in care planning, coordination, and implementation to ensure quality patient care.
- Educate patients and families on self-care techniques and assess responses to care.
- Maintain effective communication with patients, families, co-workers, and other healthcare professionals.
- Implement infection control measures to protect staff and patients.
