LPN/LVN – Home Health

**Assignment Details**
Contract Length: 13 weeks
Guaranteed Hours: 40
**Requirements**
- Current Nursing License in the Practicing State
- Valid Driver's License
- Current CPR Certification
- Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
- Proficiency in using electronic medical charting software
**Roles & Responsibilities**
- Administer skilled nursing care to patients in their residence as prescribed by the physician.
- Assist in identifying and assessing patients' physical, psycho-social, and environmental needs under the direction of the RN.
- Cooperate with the RN in planning, coordinating, and implementing patient care to ensure quality and effective outcomes.
- Communicate effectively with patients, families, and the healthcare team regarding care and outcomes.
- Educate patients and families on self-care techniques and assess their responses to the care provided.
- Adhere to infection control protocols to ensure the safety of both patients and staff.
- Support end-of-life care for Hospice patients when required.
