Position Summary

The Telemetry Registered Nurse (RN) is responsible for delivering comprehensive, patient-centered nursing care to adult patients requiring continuous cardiac monitoring. Utilizing the nursing process, the Telemetry RN performs assessments, develops individualized plans of care, administers treatments, interprets cardiac rhythms, collaborates with interdisciplinary teams, and promotes positive patient outcomes while adhering to Kaiser Permanente's clinical standards, policies, and evidence-based practices.

Essential Responsibilities

• Perform comprehensive nursing assessments and prioritize patient care based on acuity.
• Continuously monitor and interpret telemetry strips and cardiac rhythms.
• Recognize and respond rapidly to cardiac arrhythmias and changes in patient condition.
• Notify physicians and Rapid Response Teams of significant clinical changes.
• Administer medications, IV therapies, blood products, and cardiac medications safely.
• Initiate appropriate nursing interventions for deteriorating patients.
• Care for patients with:
  • Congestive Heart Failure (CHF)
  • Myocardial Infarction (MI)
  • Cardiac arrhythmias
  • Chest pain
  • Post-cardiac catheterization
  • Hypertension
  • Stroke/TIA
  • Sepsis
  • Respiratory disorders
  • Diabetes
  • Renal disease
• Assess and manage pain utilizing evidence-based interventions.
• Develop and update individualized nursing care plans.
• Educate patients and families regarding disease processes, medications, discharge planning, and lifestyle modifications.
• Coordinate care with physicians, case managers, pharmacists, respiratory therapists, rehabilitation staff, and ancillary departments.
• Maintain accurate and timely documentation within the electronic medical record (Epic).
• Participate in interdisciplinary rounds and discharge planning.
• Delegate patient care appropriately to LVNs and nursing assistants while maintaining accountability.
• Maintain infection prevention and patient safety standards.
• Ensure compliance with Joint Commission, CMS, California Board of Registered Nursing, and Kaiser Permanente policies.
• Participate in quality improvement initiatives, staff education, and competency validation.
• Function as Charge Nurse and preceptor when assigned.
